Agreed; most of the real -cities- here in the US are nowhere near old enough for a 'below' to exist as it does in London. Washington DC, for instance, is built on a -swamp-.
But there are still areas that take the place of the 'below'... I've lived and worked and played in some of them, being one of the 'Twixt that are mentioned. Because of my ample-even-for-a-technical-mundane mechanical skills, I manage to at least give the appearance of living a normal Above life.
But beyond that (beneath, if you will), are the folk sthat work the 'fringe' occupations: rennies, carnies, roadies, hobos, & gypsies... just to name a few. I've the blood of the latter, and lived the life of the first three at various points. 'America Below' is not in a cavern or tunnel under a city; it migrates about, existing in campgrounds and quiet off-the-road spots and disused warehouse districts. While it lacks in age, it's just as vital... and just as invisible to the by and large populace.
